Two Programs, One Coordinated Plan

Plenty of Trenton households are enrolled in Medicare and also in NJ FamilyCare, the state's Medicaid coverage. Juggling two programs can mean extra cards, extra paperwork and confusion about who pays what.

D-SNPs are Medicare Advantage plans designed to coordinate both programs in one place, often with very low costs for members.

What a D-SNP Usually Includes

  • Hospital, medical and prescription coverage through one plan
  • Small or zero copays on much of your care
  • A coordinator on staff to schedule visits and line up support
  • Extras such as dental, eyewear, hearing aids or an allowance for everyday health items, varying by plan
  • Help connecting to home and community-based support when you qualify

Who Can Join

Typically you need Parts A and B, full NJ FamilyCare benefits and a home address in the plan's service area, which includes Mercer County for several carriers. Anyone receiving only partial state help, for example through a Savings Program, is usually steered toward other plan types.

Changing Plans

Dual eligibles have more flexibility than most Medicare members. Since 2025, people with both programs can generally switch once a month into a plan that integrates Medicare and Medicaid, or drop a Part C plan in favor of Original Medicare with a stand-alone drug plan.

Keeping Your Doctors

As with any Part C plan, D-SNPs use networks. Before you join, we confirm that the physicians, specialists and drugstores you rely on are in network, and whether providers you see at Capital Health or in Hamilton are included.

A Free Second Opinion on Your Coverage

Jot down your prescriptions and the names of the doctors you see. We line up what you have now against the plans we represent and point out the real differences in cost and in where you can get care. Expect about thirty minutes, with no decision required during the call. You can also read our background before calling.

What Trenton Neighbors Ask About D-SNPs

Would joining a D-SNP end my NJ FamilyCare?

It would not. Your Medicaid benefits continue; the plan simply coordinates them with Medicare.

Will I pay a premium?

Many members pay nothing for the plan itself, but we confirm costs for each option.

What if I only get help with my Part B premium?

You may qualify for other options. We review your letters from the state to see which plans fit.
